• Terms of Use Letter - Effective, November 1, 2006. For new patients, Fairfax Cryobank must receive a signed copy of this Terms of Use letter via fax, mail or scanned image into email at least one business day prior to shipment of any Specimens. This document explains what you can expect from our program and what we can expect from you when purchasing our semen and other services.
• Must Read Before You Buy- gives advice on commonly asked about topics including donor availability, our specimen warranty and our limits on donor distribution.
• Privacy Policy offers our assurances that we will not compromise donors' or our clients' identities.

• Photo Match Request Form- for the photo matching service
• Identity (ID) Consent Donors Form- must review instructions and then download the form
• Identity (ID) Consent Birth Registration Form- must be completed after the birth of a child from a ID Consent donor if access to donor information at age 18 is desired
• Lifetime Photo Consent Form- required before we will mail your order
